Health Plan Benefits

We encourage you to review your coverage prior to your visit to The Endoscopy Center of Fairfield. We believe that it is important for you to understand the extent of your health insurance coverage and the responsibilities you have as part of that coverage. Many plans include co-payment, deductible and coinsurance requirements and have limitations on how you can access services you may need.

The Endoscopy Center of Fairfield participates with all major insurance companies. Prior to your appointment, please check with your insurance carrier to verify our participation with your specific insurance plan.

Referrals and Authorizations

Prior to your visit to our facility, please review your member handbook or contact your member services department to be sure your referral/authorization has been obtained. There will be services that will require authorizations that only we can obtain. We will work collaboratively with your insurance carrier to ensure that all the requirements for authorization are met by the time of your visit. This is not a guarantee of payment. Payment will be determined by your insurance.

Co-Payments

Please be prepared to pay your co-payments at the time of each visit. Often the co-pay requirements are outlined on your insurance card. If they are not, please contact your member services department prior to your visit. For your convenience we accept cash, checks and all major credit cards.
